This disturbing clip is from the Spanish short Días sin luz (1995) an early work by Jaume Balagueró who went on to some renown in horror circles for the [Rec] series.  You could describe it as "David Lynch directs a Nine Inch Nails" video and not be far off. There is something distinctly Spanish about it and it shares some similarities to "Vintage" Dominatrix and Boots, but this is more distinctly of the horror genre.
